A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes

 

Here's the Scoop:

To participate in the Quality Insights and PA DOH's A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes initiative, your practice must agree to do the following:

  • Designate an individual to act as the primary contact for Quality Insights.
  • Treat patients at risk for or have diabetes.
  • Submit annual quality reports for National Quality Forum (NQF) #0059- Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9%).
  • Participate in a workflow assessment and analysis regarding current prediabetes and diabetes processes within the practice.
  • Receive quality improvement recommendations and assistance from your Quality Insights Practice Transformation Specialist.
  • Consider best practice workflow modifications for implementation within your office.

Are You Monitoring and Submitting Your Practice's NQF #0059 Measure?

One of the top reasons for you and Quality Insights to monitor NQF data is to ensure you are providing optimal care to your patients. Additionally, monitoring your data can assist your practice and Practice Transformation Specialist in identifying opportunities for improvement.

Other reasons include:

  1. External importance for public and payer reporting and payment reform.
  2. Consistency with/comparable to existing national measure sets.
  3. Allows for consistent comparison of primary care provider organizations or within Accountable Care Organizations (ACOs).
  4. A tool to support clinical quality improvement initiatives.
  5. A tool to support additional financial benefits.
  6. Implement processes already established and tested by others.
