Let's Work Together to Combat Diabetes  

According to “The Burden of Diabetes in Pennsylvania” and the Centers for Disease Control and Prevention (CDC), “over 37 million Americans have diabetes and face its devastating consequences. What’s true nationwide is also true in Pennsylvania.” The statistics are staggering. The incidence of diabetes in the state of Pennsylvania and across the country continues to rise.

Practices treat many patients with prediabetes and diabetes on a daily basis.  Like most clinicians, your resources are limited, and you need to maximize what you have. However, many of the risks of diabetes are preventable and controllable.

Quality Insights, under an agreement with the Pennsylvania Department of Health (PA DOH), offers NO COST ASSISTANCE to eligible healthcare systems and independent private practices to advance the aims of the Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes initiative throughout Pennsylvania. This project focuses on:

  • Preventing or delaying the onset of type 2 diabetes among adults with prediabetes.
  • Supporting the  implementation of evidence-based, family-centered childhood obesity interventions as a type 2 diabetes risk reduction strategy
  • Improving self-care practices, quality of care, and early detection of complications among people with diabetes.

  • Optimize the use of your electronic health record (EHR)/health information technology (HIT)

  • Integration of data and quality outcomes to support value-based care and enhancing Quality Payment Programs and PA Promoting Interoperability

How It Works

  • Practices will receive assistance from Quality Insights experts at no cost to them.
  • Onsite and virtual assistance is available, plus phone and email support.
  • A Memorandum of Understanding (MOU) and Business Associate Addendum (BAA) is signed that outlines the role of the practice and Quality Insights.
