Welcome to the Quality Insights National Cardiovascular Health Program
Practices treat many patients with hypertension and/or hypercholesterolemia on a daily basis. Like most clinicians, your resources are limited, and you need to maximize what you have.
According to the Centers for Disease Control and Prevention (CDC), about 1.5 million heart attacks and strokes occur every year in the United States. Risks for heart disease and stroke include high blood pressure and high cholesterol and do not present any symptoms. However, many of these risks, specifically hypertension, high cholesterol, and smoking, are preventable and controllable.
Controlling these risks could reduce a person's risk for heart attack or stroke by more than 80%.
Our goal at Quality Insights is to arm Pennsylvania healthcare providers with the evidence-based education, guidance, and resources you need to help patients improve cardiovascular control and prevention.
Quality Insights, under an agreement with the Pennsylvania Department of Health (PA DOH), continues to offer NO COST ASSISTANCE to eligible healthcare systems and independent private practices to improve individual patient care and overall population health for chronic diseases.

- Workflow assessment and analysis
- Analysis of process and outcome measures
- Optimize the use of your electronic health record (EHR)/health information technology (HIT)
- Integration of data and quality outcomes to support value-based care and enhancing Quality Payment Programs and PA Promoting Interoperability
- Education resources for staff and patients
- Quality improvement guidance
How It Works
- Practices will receive assistance from Quality Insights experts at no cost to them.
- Onsite and virtual assistance is available, plus phone and email support.
- A Memorandum of Understanding (MOU) and Business Associate Addendum (BAA) is signed that outlines the role of the practice and Quality Insights.
