Improving Hypertension and Diabetes Control & Prevention in Delaware

 

Here's the Scoop:

To participate in the Quality Insights and Delaware Division of Public Health's Implementation of Quality Improvement Initiatives to Improve Diabetes and Hypertension project, your practice must agree to do the following:

  • Designate an individual to act as the primary contact for Quality Insights.

  • Treat patients with hypertension or diabetes.

  • Submit quarterly and annual quality reports for NQF #0018 (Blood Pressure in Control), CMS 347 or ACO 42 (Cardiovascular patients prescribed a statin). 

  • Submit baseline data for NQF #0018, NQF #0059 (HBA1C Poor Control), and CMS 347 or ACO 42.

  • Participate in a workflow analysis regarding current hypertension and diabetes processes with the practice.

  • Receive quality improvement recommendations and assistance from your Quality Insights Practice Transformation Specialist.

  • Consider best practice workflow modifications for implementation within your office.

  • Share provided education materials with your staff and, as appropriate, patients.

Are You Monitoring and Submitting Your Practice's NQF #0018, NQF #0059, and CMS 347/ACO 42 Measures?

One of the top reasons to monitor and submit your National Quality Forum (NQF) data is to assure you are providing optimal care to your patients.  Additionally, monitoring your data on a frequent basis can assist your practice in identification of opportunities for improvement.

Other reasons include:

  1. External importance for public reporting and payment reform

  2. Consistency with/comparable to existing national measure sets

  3. Allows for consistent comparison of primary care provider organizations or within Accountable Care Organizations (ACOs)

  4. A tool to support clinical quality improvement initiatives

  5. A tool to support additional financial benefits

  6. Implement processes already established and tested by others
