Welcome to Our 2024 Program Year!
The 5 Diamond Patient Safety Program was developed in 2008 for dialysis providers and other healthcare staff to assess and improve their culture of patient safety within their dialysis facilities.
The program is an automated, online, national educational resource, consisting of patient safety topic modules including the tools and resources necessary to implement each patient safety concept. Any dialysis facility, regardless of Network area may participate in the program. For each module successfully completed during a program year, the facility earns one Diamond. Upon successful completion of five modules, and a Program Review questionnaire completed by all staff, the facility is recognized as a 5-Diamond Patient Safety Facility.
We are excited to launch the 17th program year which is free to every dialysis facility in the nation, with full continuing education credit available. For instructions, tips and tricks to using the new platform check out the 5 Diamond User Guide.

Upon completion of this module, dialysis facility personnel will be able to:
- Describe the basic concepts of patient safety in the healthcare environment
- Identify best practices to prevent safety errors
Care Coordination/Care Transitions
Care Coordination/ Transitions
Upon completion of this module, dialysis facility personnel will be able to:
- Define care coordination and care transitions
- Discuss challenges associated with care coordination
Home Dialysis Therapy
Home Dialysis Therapy
Upon completion of this module, dialysis facility personnel will be able to:
- Educate all eligible patients about home dialysis therapy
- Assess all eligible patients for interest, and refer as appropriate, all eligible patients to home dialysis therapy
Transplantation
Upon completion of this module, dialysis facility personnel will be able to:
- recognize transplantation as the preferred modality for dialysis patients with ESRD
- implement clearly defined education and referral protocols that utilize a facility-level transplant liaison in transplantation discussions with patients and staff coordination
Vaccinations
Upon completion of this module, dialysis facility personnel will be able to:
- identify recommended vaccinations for dialysis patients
- discuss myths vs. facts related to vaccinations
