Nursing Home 5-Diamond Program
Demonstrate Your Facility’s Commitment to Patient Safety
Patient Safety is the Goal
The Nursing Home 5-Diamond Program from Quality Insights is an automated, online, national educational resource for patient safety in nursing homes. The program modules focus on appropriate medication use, rehospitalization reduction, infection control, and more.
Each module includes the tools and resources necessary to implement a specific patient safety concept.

Who Can Participate?
Any participating long-term care facility may enroll in the program. For each module successfully completed, the facility earns one Diamond.
Upon successfully completing five modules and completing a Program Review questionnaire by all staff, the facility will earn “5-Diamond Patient Safety Facility” recognition.
This education is appropriate for all nursing home staff, including Directors of Nursing, Nursing Home Administrators, Social Workers, Frontline Staff, and Medical Directors.

NO-COST Continuing Education Credits
Several of the modules in the Nursing Home 5-Diamond Program offer NO-COST continuing education (CE) credits.
